Double board-certified Houston plastic surgeon combines classical sculpture, advanced body-contouring techniques and published research on large-volume liposuction safety
HOUSTON, Sept. 3, 2026 /PRNewswire/ — Long before a patient enters the operating room, plastic surgeon Emmanuel De La Cruz, MD, is thinking in three dimensions.

Beyond Simply Removing Fat
For De La Cruz, modern liposuction is not simply a matter of removing as much fat as possible.
In high-definition body contouring, the objective is to selectively modify the thickness of the adipose layer while respecting the underlying muscular anatomy and the patient’s natural proportions.
That distinction, he says, separates fat removal from body sculpting.
“If you remove fat without understanding the anatomy underneath it, you may make someone smaller, but that doesn’t necessarily mean you’ve created a better shape,” De La Cruz said. “Body contouring is about proportion. Sometimes a few millimeters in the right location can be more important aesthetically than removing another liter of fat.”
His practice incorporates techniques including VASER-assisted liposuction, power-assisted liposuction and, when appropriate, energy-based technologies designed to complement surgical body contouring.
But De La Cruz cautions that increasingly sophisticated technology should never become a substitute for surgical judgment.
“A device is a tool. It doesn’t select the patient, understand anatomy, manage fluid shifts or recognize a developing complication. Technology can enhance what we do, but the surgeon and the surgical team remain responsible for the operation.”
Bringing Patient Safety Into the Conversation
De La Cruz has also contributed to the medical literature on a subject receiving increasing attention as body-contouring procedures become more extensive: large-volume liposuction and the overweight patient.
His peer-reviewed article, “Lipoplasty in the Overweight Patient,“ published in Clinics in Plastic Surgery, examines patient selection and perioperative considerations surrounding larger-volume procedures, including fluid management, prevention of hypothermia, venous thromboembolism prophylaxis and postoperative monitoring.
His central message is that the safety of liposuction cannot be reduced to a single number.
“People naturally focus on how many liters were removed or how many hours an operation lasted, but surgical safety is much more complex than that,” De La Cruz said. “You have to consider the patient, the magnitude of the operation, anesthesia, fluid management, temperature control, VTE prevention, the surgical facility and the level of postoperative monitoring together.”
For extensive procedures, De La Cruz advocates matching the surgical environment and postoperative resources to the magnitude of the operation.
“The larger and more physiologically demanding the procedure becomes, the more important the infrastructure surrounding that patient becomes,” he said. “The right patient, the right procedure, the right surgical and anesthesia team, and the right facility all have to come together.”
Rejecting the “One-Look” Era of Plastic Surgery
At a time when social media can rapidly popularize a particular waistline, silhouette or body type, De La Cruz believes plastic surgery should move in the opposite direction.
He rejects the concept that every patient visiting a particular surgeon should emerge with a recognizable “signature” body.
“I don’t believe in giving everyone the same look,” De La Cruz said. “Every person walks into my office with different anatomy, proportions and goals. My responsibility is to understand what is already there and determine how surgery can enhance it—not erase their individuality.”
It is a philosophy he summarizes simply:
“Not one look. Your look.“
The Future of Plastic Surgery May Be More Human, Not Less
As artificial intelligence, imaging systems and increasingly sophisticated surgical technologies enter aesthetic medicine, De La Cruz believes the next evolution of plastic surgery will depend on combining technology with something far older: an understanding of human form.
“We have extraordinary technology available to us today, and it will continue to improve,” he said. “But plastic surgery is still an art grounded in anatomy. You need science to operate safely, technology to expand what is possible, and an artistic understanding of proportion to know when you’ve done enough.”
