Report analyzes 12 months of provider directory audits and millions of provider records to identify systemic data quality challenges and outline a roadmap for operational improvement.

LANGHORNE, Pa., Sept. 3, 2026 /PRNewswire/ — Perspecta today announced the release of The State of Provider Data Accuracy, a comprehensive research report examining data quality challenges across U.S. health plan provider directories. Drawing on insights from 12 months of provider directory audits and millions of provider records, the report details the root causes of provider data inaccuracies and their impact on regulatory compliance, operational performance, and member trust.

Industry benchmarks indicate average provider data accuracy hovers around 50% and only 31% of members report trusting their health plan to help them find the right provider.

“Provider data has become one of the most important operational assets for health plans, yet it’s also one of the most difficult to manage,” said Mark Stamper, Director of Data Operations, at Perspecta. “The findings confirm that provider data decay is not a periodic issue, but a continuous data management challenge that requires systematic, ongoing oversight.”

Key findings highlighted in the report include:

  • High-Risk Data Fields: Five core fields consistently account for the vast majority of directory errors: address, location name, phone number, fax number, and specialty.
  • Limitations of Traditional Validation: Standard periodic attestation cycles and manual outreach fail to keep pace with continuous real-world changes in provider information.
  • Downstream Operational Impacts: Inaccuracies directly affect regulatory exposure, claims processing efficiency, credentialing workflows, network adequacy monitoring, and overall member experience.
  • Pathways to Sustainable Quality: Implementing continuous, systematic data auditing enables health plans to systematically reduce error rates and maintain long-term compliance.

The report offers practical framework recommendations to help health plans benchmark current data accuracy, prioritize remediation for high-risk fields, and establish scalable data governance models. By combining broader industry metrics with operational audit findings, the study provides a practical, data-grounded perspective on modernizing provider directory management.

“With regulatory standards tightening and member expectations rising, provider data accuracy is a fundamental requirement for healthcare operations,” added Stamper. “This research aims to give health plan leaders actionable visibility into where data breaks down and how to establish sustainable accuracy.”
