AMCP Market Insights Findings Show Uncertainty in Biomarker Testing for ESR1-Mutated Advanced Breast Cancer as Payers Weigh Earlier Detection and a Growing Class of Targeted Therapies

WASHINGTON, Oct. 16, 2026 /PRNewswire/ — Breast cancer is the most common cancer diagnosed in U.S. women. In hormone receptor positive, HER2 negative advanced cases, resistance to first line endocrine therapy is common. An estrogen receptor alpha gene mutation, ESR1m, drives resistance in nearly half of these patients, leaving them on ineffective treatment until radiologic progression is detected.

A new AMCP Market Insights Program, “Managed Care Approaches to ESR1-Mutated Advanced Breast Cancer,” finds that biomarker testing and treatment paradigms are evolving faster than the evidence needed to manage them. Monitoring ESR1m prior to radiologic progression using circulating tumor DNA (ctDNA) liquid biopsy can enable an early switch to effective therapy timely treatment adjustment in order to extend time on first-line therapy. However, uncertainty around optimal testing parameters drives inconsistent testing practices, coverage friction across medical and pharmacy benefits, and care delays.

AMCP identified clear opportunities for payers to close these gaps. Recommendations include implementing flexible, evidence-based treatment pathways, aligning coverage policies across benefits, and streamlining prior authorization through auto-approvals for guideline-concordant testing, extended authorization durations, and gold carding. Payers also emphasize the need for total cost-of-care analyses that account for delayed progression rather than testing costs alone, alongside improved support for community oncologists and patient shared decision-making.

As next-generation selective estrogen receptor degraders move into earlier treatment lines, promoting testing consistency and multi-stakeholder alignment represents a critical opportunity to improve patient outcomes.

This program was independently conducted with funding support from AstraZeneca. The full peer-reviewed Market Insight supplement is published in the October 2026 edition of the Journal of Managed Care & Specialty Pharmacy.
