Centennial, Colorado – Pinnacle Holdings, LTD (“Pinnacle”) provides healthcare consulting services to certain healthcare organizations. Pinnacle experienced a data event that impacted certain information related to patients affiliated with those healthcare organizations.
On November 25, 2024, Pinnacle experienced a network disruption that impacted certain systems. Upon discovery, Pinnacle took immediate action to address and investigate the event, which included engaging third-party specialists to assist with determining the nature and scope of the incident. The investigation determined that limited information maintained on Pinnacle’s network may have been acquired by an unauthorized actor between November 11, 2024, and November 25, 2024. The information varied by individual but may include included a patient name, address, phone number, email address, Social Security number, driver’s license/state ID number, taxpayer ID number, passport number, financial account information, payment card information, online account credentials, digital signature, biometric data, date of birth, medical treatment or diagnosis information, prescription information, date of service, patient ID number, encounter ID number, provider name, patient account number, medical record number, Medicare/Medicaid number, health insurance information, health insurance claim number, health insurance policy number, and/or treatment cost information.
Pinnacle immediately undertook a thorough review of the information to identify potentially affected individuals. Pinnacle then worked diligently with its healthcare partners to identify contact information for individuals to notify them of the incident, and Pinnacle is mailing letters to individuals for whom it has address information.
Pinnacle has also established a toll-free call center to answer questions about the incident, provide access to complimentary credit monitoring services, and address related concerns. The call center is available between 9:00 AM and 6:30 PM Eastern Time, Monday through Friday, excluding major U.S. holidays and can be reached at 1-866-686-2607. Pinnacle also implemented additional security measures to help prevent a similar occurrence in the future.
Individuals are encouraged to remain vigilant against incidents of identity theft and fraud by reviewing credit reports, account statements, and explanation of benefits forms for suspicious activity and to detect errors. Individuals may also place a fraud alert or credit freeze by contacting the credit reporting agencies: TransUnion at 1-800-680-7289; Experian at 1-888-397-3742; Equifax at 1-888-298-0045.
